Membership Application & Conference Registration
You can easily pay for your yearly membership, Conference and Course registration
through our convenient online ordering process: Down load the appropriate form, fill out and email to hburns@ssmgt.com Then To pay on line: Click the catagory below to purchase: Membership costs ◊Individual Membership for Clinicians (MDs and PhDs) with full voting rights is $100/year ◊Associate Membership for Technicians and Students with no voting rights is $50/year ◊MEG center Membership for the Director + Administrator with full voting rights is $2,000/year (please contact us for invoice) Meeting costs - $150.00 This includes Dinner and conference materials
◊Individual Members from a Center MEG membership site are eligible for a 50% discount ($75)
◊Associate Members are eligible for a 50% discount ($75) ◊Nonmembers who elect not to go to
the dinner are eligible for a $75 reduction in the cost. Course
costs - $200.00 This includes Breakfast, Lunch and CME credits ◊Individual Members from a Center MEG membership
site are eligible for a 25% discount ($150) ◊Associate Members are eligilbe

Annual meeting please let us know if you will be coming to Dinner and if you will be bringing a guest. Dinner for guest
